If you’d prefer a paper copy, give us a call and we’ll send you one. Once filled in please fax (02 6792 4582), or mail it back to us (2MaxFM PO Box 297, Narrabri, NSW, 2390), or drop it in at the station (73 Barwan St, Narrabri) before the cut-off date.
